Customer VFD Forms

For Our Customers that Will Be Distributing VFD Products After January 1, 2017:

All of our customers that plan on purchasing VFDs or medicated feeds containing a VFD drug for distribution must fill out the forms attached to this page. The forms below are the same forms that we have already mailed out to our current customers. If you do not fill out and send in the forms you will not be able to purchase any products labelled as a VFD after the new FDA regulations are in place on January 1, 2017.
